Welcome to the M Health Fairview Athletic Training scheduling system.
This system is designed to help you to conveniently post your needs for athletic training coverage.
We typically schedule athletic trainers three to four months out.
The sooner you post your request, the more likely we will be able to cover your event. Click on "new request" to post an event or tournament.
Please post multiple dates at the same venue on one request form, using the "add" button below the date field. Post multiple venues on separate requests. Once you have posted all your needs, a confirmation or denial will be returned via email. The confirmation only confirms that your event is posted and allows our staff the opportunity to view the needs and sign up to cover your event. If we cannot guarantee coverage, someone will contact you to give you varying options for medical coverage, including partial coverage. If event times change, we require a two-week notice to adjust staff schedules without penalty.
M Health Fairview's athletic trainers bring their own supplies to events. If taping is expected, teams should provide tape. Ice should be supplied by the event or tournament. Our athletic trainers will typically arrive 15 minutes prior to posted time.

About our Staff:
Our athletic trainers are nationally certified and state licensed health care professionals that provide acute medical care ranging
from concussions to weather related illness and all the bones and muscles in between. Our athletic trainers also manage appropriate return to play
decisions for musculoskeletal injuries and concussions.
Athletic trainers on site will also work with athletes and families in navigating further medical care.

Additional Information
Bulk supplies are available at an additional cost of current market value and requires advanced notice to order additional supplies. Invoices will be sent out within 3 weeks upon the completion of the event. Invoices will include time of arrival of our athletic trainers through the time of their departure from the event.

The hourly rate includes:
- One athletic trainer per sheet of ice OR
- One athletic trainer per 4 mats of wrestling OR
- One athletic trainer per dual track meet OR
- One athletic trainer per soccer / football / lacrosse game OR
- One athletic trainer per venue of non-collision events
- Holidays include: New Year’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day and Christmas Day.
Late request service fee of $60.00
- Athletic training requests submitted within 3 weeks of a requested coverage date are subject to this administrative fee, per requested coverage date.
- If last minute changes occur that result in cancellation or rescheduling, please contact the athletic trainer assigned to you as soon as possible. If you have not contacted our staff and they show up to a canceled event, you will be charged the same administrative fee.
